ABSTRACT

Objective:To evaluate the efficacy and safety of mitoxantrone hydrochloride liposome injection in the treatment of peripheral T-cell lymphoma (PTCL) in a real-world setting. Methods:This was a real-world ambispective cohort study (MOMENT study) (Chinese clinical trial registry number: ChiCTR2200062067). Clinical data were collected from 198 patients who received mitoxantrone hydrochloride liposome injection as monotherapy or combination therapy at 37 hospitals from January 2022 to January 2023, including 166 patients in the retrospective cohort and 32 patients in the prospective cohort; 10 patients in the treatment-na?ve group and 188 patients in the relapsed/refractory group. Clinical characteristics, efficacy and adverse events were summarized, and the overall survival (OS) and progression-free survival (PFS) were analyzed. Results:All 198 patients were treated with mitoxantrone hydrochloride liposome injection for a median of 3 cycles (range 1-7 cycles); 28 cases were treated with mitoxantrone hydrochloride liposome injection as monotherapy, and 170 cases were treated with the combination regimen. Among 188 relapsed/refractory patients, 45 cases (23.9%) were in complete remission (CR), 82 cases (43.6%) were in partial remission (PR), and 28 cases (14.9%) were in disease stabilization (SD), and 33 cases (17.6%) were in disease progression (PD), with an objective remission rate (ORR) of 67.6% (127/188). Among 10 treatment-na?ve patients, 4 cases (40.0%) were in CR, 5 cases (50.0%) were in PR, and 1 case (10.0%) was in PD, with an ORR of 90.0% (9/10). The median follow-up time was 2.9 months (95% CI 2.4-3.7 months), and the median PFS and OS of patients in relapsed/refractory and treatment-na?ve groups were not reached. In relapsed/refractory patients, the difference in ORR between patients with different number of treatment lines of mitoxantrone hydrochloride liposome injection [ORR of the second-line, the third-line and ≥the forth-line treatment was 74.4% (67/90), 73.9% (34/46) and 50.0% (26/52)] was statistically significant ( P = 0.008). Of the 198 PTCL patients, 182 cases (91.9%) experienced at least 1 time of treatment-related adverse events, and the incidence rate of ≥grade 3 adverse events was 66.7% (132/198), which was mainly characterized by hematologic adverse events. The ≥ grade 3 hematologic adverse events mainly included decreased lymphocyte count, decreased neutrophil count, decreased white blood cell count, and anemia; non-hematologic adverse events were mostly grade 1-2, mainly including pigmentation disorders and upper respiratory tract infection. Conclusions:The use of mitoxantrone hydrochloride liposome injection-containing regimen in the treatment of PTCL has definite efficacy and is well tolerated, and it is a new therapeutic option for PTCL patients.

ABSTRACT

Chronic kidney disease (CKD) is a global health problem, and its incidence increases year by year. Studies have revealed that the progression of CKD into end-stage renal disease (ESRD) is related to its inability to effectively eliminate toxins due to decreased renal function. Additionally, intestinal microflora produces a large amount of gut-derived uremic toxins (GDUTs) during protein fermentation. The theory of gut-kidney axis holds that gut and kidney interact with each other, and CKD reduces the ability to remove uremic toxins (UTs), resulting in the accumulation of UTs in the blood. The accumulation of UTs also accelerates the deterioration of renal function, leading to a vicious circle. This paper focused on the sources of indoxyl sulfate and p-cresol sulfate in GDUTs and their mechanisms against CKD (such as inducing renal tubular cell death, oxidative stress and endothelial injury, promoting renal fibrosis and down-regulating renal protective protein) as well as the sources of trimethylamine oxide and its mechanisms against CKD (such as promoting renal fibrosis and inflammation). Moreover, starting from gut-kidney axis, this paper summarized the ways of diet and nutrition regulation, toxin adsorption, enhanced dialysis to increase the clearance, inhibiting the sources of gut-derived toxins and traditional Chinese medicine (TCM) therapy (TCM preparations and TCM active ingredients) to regulate intestinal microecology and reduce the generation of GDUTs, aiming to provide new therapeutic ideas for delaying the progression of CKD.

ABSTRACT

Objective:To retrospectively analyze clinical efficacy and safety of omalizumab in the treatment of chronic urticaria (CU) in southern Zhejiang, China. Methods:A retrospective observational study was conducted on CU patients who received omalizumab treatment at the First Affiliated Hospital of Wenzhou Medical University from January 1st, 2018 to August 1st, 2021. Through the outpatient follow-up visits, the disease activity, condition control, and quality of life were evaluated using the 7-day urticaria activity score (UAS7) , urticaria control test (UCT) , and dermatology life quality index (DLQI) . In addition, changes in disease condition, recurrence after withdrawal, and adverse events were assessed. Independent-sample t test was used for intergroup comparisons of normally distributed measurement data, Wilcoxon signed-rank sum test or Kruskal-Wallis H test was used for comparisons of non-normally distributed measurement data, and chi-square test or Fisher′s exact test was used for comparisons of enumeration data. Results:A total of 252 CU patients with poor response to antihistamines were included, with a baseline UCT score of 5.0 ± 2.4 points, a UAS7 score of 25.6 ± 6.2 points, and a DLQI score of 17.5 ± 4.7 points; among them, 204 (81.0%) were treated with omalizumab at an initial dose of 300 mg, and 48 (19.0%) with omalizumab at an initial dose of 150 mg. At the end points (12.0 ± 1.4 months after the start of treatment) , an overall control rate of 90.3% (224/248) was achieved after the omalizumab treatment; concretely, 137 (55.2%) patients achieved complete control (UCT = 16 points) , 87 (35.1%) achieved partial control (12 points ≤ UCT < 16 points) , and 24 (9.7%) showed no response (UCT < 12 points) , while 10 with partial response shifted to complete control after dose increase. During the treatment period, recurrence occurred in 50 patients (36.5%) , of whom 32 patients opted for retreatment with omalizumab, and then 30 (93.8%) achieved partial or complete control. Adverse events were reported in 8 patients (3.2%) , and all were mild or moderate. Conclusion:Omalizumab was effective in the real-world treatment of CU, and could improve patients′ quality of life, with a favorable safety profile.

ABSTRACT

OBJECTIVE: To investigate the improvement effects of water extract of Astragalus membranaceus on chronic renal failure (CRF) model rats and its effects on MAPK signaling pathway, and to investigate possible mechanism. METHODS: Male SD rats were randomly divided into control group (10 rats) and model group (50 rats). CRF model was established by intragastric administration of 25% Adenine suspension 200 mg/kg (once a day, for consecutive 28 d). After modeling, modeling group was randomly divided into model group, benazepril group (positive control, 2 mg/kg), A. membranaceus water extract low-dose, medium-dose and high-dose groups (1.5, 3, 6 g/kg,by crude drug), with 10 rats in each group. Control group and model group were given constant volume of normal saline intragastrically, and administration groups were given relevant medicine intragastrically, once a day, for consecutive 28 d. Twelve hours after last medication, the contents of serum renal function indexes (serum creatinine, urea nitrogen, uric acid) were determined by colorimetry. ELISA method was used to detect the levels of serum inflammatory factors (IL-1β, IL-6 and TNF-α). The activity or content of oxidative stress related indexes (SOD, CAT and MDA)in renal tissue of rats were determined by hydroxylamine method, visible spectrophotometry method and thiobarbituric acid method. The mRNA expression of apoptosis-related factors (Bax, Bcl-2, Caspase-3) in renal tissue, MAPK signaling pathway-related regulatory protein p38 MAPK, phosphorylated p38 MAPK(p-p38 MAPK), ERK1/2, phosphorylated ERK1/2 (p-ERK1/2), JNK and phosphorylated JNK (p-JNK) were determined by real- time PCR and Western blotting assay. RESULTS: Compared with control group, the contents of renal function indexes and the levels of inflammatory factors in serum, MDA content, mRNA expression ratio of Bax and Bcl-2 (Bax/Bcl-2), mRNA related expression of Caspase-3, related expression of phosphorylation product of MAPK signaling pathway-related regulatory proteins (p-p38 MAPK, p-JNK, p-ERK1/2) in renal tissue were increased significantly in model group, while the activities of SOD and CAT were decreased significantly in renal tissue (P<0.01). Compared with model group, the contents of renal function indexes and the levels of inflammatory factors in serum, Bax/Bcl-2, related expression of phosphorylation product of MAPK signaling pathway-related regulatory proteins in renal tissue were decreased significantly in administration groups as well as MDA content and mRNA related expression of Caspase-3 were decreased significantly in benazepril group and A. membranaceus water extract medium-dose and high-dose groups; the activities of SOD and CAT in renal tissue were increased significantly in benazepril group and A. membranaceus water extract medium-dose and high-dose groups (P<0.05 or P<0.01). MDA content and Bax/Bcl-2 of benazepril group were significantly lower than those of A. membranaceus water extract high-dose group (P<0.05). CONCLUSIONS: A. membranaceus water extract has a certain improvement effect on CRF model rats, and inhibit its inflammatory reaction, oxidant stress and cell apoptosis, the mechanism of which may be associated with inhibiting the expression of MAPK signaling pathway-related regulatory proteins.

ABSTRACT

Extranodal natural killer (NK)/T-cell lymphoma, nasal type (ENKL) is an uncommon subtype of non-Hodgkin's lymphoma, which is aggressive and has poor prognosis. ENKL occurs predominantly in the nasal cavity and less frequently in the skin and in the gastrointestinal tract. The pathology of this disease is characterized by vascular invasion and destruction. ENKL is strongly associated with the Epstein–Barr virus, which aids in the diagnosis of ENKL. The characteristic markers on the surface of an NK/T cell and the spe-cific genetic variations are also useful in the diagnosis. At present, the treatment of ENKL is still under discussion. Although radiothera-py combined with chemotherapy on patients with early-stage ENKL and L-ASP-based chemotherapy and hematopoietic stem cell trans-plant on patients with advance-stage ENKL have achieved favorable effects, further studies are still necessary to develop the principles and methods of a standardized treatment.

ABSTRACT

Objective To analyze mutations in the GJB2 gene in a Chinese patient with keratitis-ichthyosis-deafness (KID)syndrome complicated by cutaneous squamous cell carcinoma. Methods Clinical data were collected from a patient with KID syndrome complicated by cutaneous squamous cell carcinoma. Peripheral blood samples were obtained from the patient and her parents, and DNA was extracted from these blood samples. PCR was performed to amplify the exon 2 of the GJB2 gene followed by direct DNA sequencing. Results A mutation (c.148G > A)was identified at position 148 in exon 2 of the GJB2 gene, which caused a codon change from GAC to AAC and resulted in the substitution of aspartate by asparagine at position 50 in the connexin26 (Cx26)protein (p.Asp50Asn). Inaddition, anothermutation(c. 79G > A), which led to the substitution of valine by isoleucine at codon 27 in Cx26 (p.Val27Ile), was found at position 79 in exon 2 of the GJB2 gene. Neither of the two mutations was detected in the patient′s parents. Literature review revealed that 13 cases of KID syndrome complicated by cutaneous squamous cell carcinoma had been reported in abroad, and the mutation c.148G > A was detected in the GJB2 gene in all the 7 cases finally diagnosed by gene sequencing. Conclusion GJB2 gene mutations may be responsible for the clinical phenotype of KID syndrome in this Chinese patient, and the mutation c.148G > A may be related to the development of cutaneous squamous cell carcinoma.

ABSTRACT

JAK/STAT includes many types of cell factors. Growth factor signal transduction is an important pathway and widely participates in cell multiplication, differentiation, and immunity adjustment. Aberrant-activated JAK/STAT signaling pathway is in-volved in carcinogenesis. Recent studies demonstrated that abnormal expression and activation of STAT3 were found in lymphoma. Constitutive activation of STAT3 promotes development, invasion, and metastasis of cancer. AG490, a JAK2 inhibitor, can block the JAK/STAT3 signal pathway and reduce the pathway's downstream STAT3 expression. Several studies showed that AG490 can inhibit cell proliferation and promote apoptosis in lymphoma. AG490 combined with chemotherapy drugs could improve sensitivity. In our study, we reviewed the potential role of JAK/STAT signaling pathway and the blocker AG490 in lymphoma.

Objective To explore the clinical efficacy of adefovir dipivoxil in treatment of liver cirrhosis. Methods 45 cases with liver cirrhosis admitted in 302 Military Hospital of Chian in 2012 were randomly divided into experiment group(n=25)and control group(n=20).Experiment group was treated with adefovir dipivoxil, and control group was given traditional therapy. Two groups were treated for six months.Renal function, liver function and liver firosis were examined with every month, the recovery indexes of renal function between two groups were detected and compared after treatment, which was used to judge the clinical efficacy of adefovir dipivoxil. Results Before treatment, there were no significant differences in the thickness of spleen, serum indexes of portal vein, splenic vein and hepatic fibrosis between experiment group and control group. After treatment, the liver and spleen condition and liver fibrosis index including HA,LN,PCIII,IV-C in two groups were significantly improved by B ultrasound and serum detection, and the improvement degree of experimental group was significantly higher than control group(P<0.05).Conclusion Adefovir dipivoxil has better therapic efficacy in treatment of liver cirrhosis.

ABSTRACT

Objective To explore the application value of 320-slice spiral CT myocardial perfusion technique in the evaluation of cardiac function in patients with coronary artery disease. Methods 24 patients with known reversible myocardial ischemia with coro-nary heart disease and 13 cases of normal were detected by 320-slice spiral CT myocardial perfusion scan in resting and after dobu-tamine-induced stress, measured the values of cardiac function indexes(ESV、EDV、SV、EF)in resting and after dobutamine-induced stress respectively, the results were analyzed by paired t test and independent sample t test, when the P value is less than 0.05,con-sidered statistically significant or correlation. Results The values of cardiac function indexes (ESV,EDV,SV)between the resting and after dobutamine-induced stress were significantly different except EF value in normal group. But only the EF value was signifi-cantly different in CHD,the rest values of cardiac function indexes(ESV,EDV,SV)no significant difference. Each corresponding value of cardiac function indexes in resting and after dobutamine-induced stress was significantly different between the CHD and the normal. Conclusion The dobutamine-induced stress myocardial perfusion with 320-slice spiral CT can judge the impaired degree of cardiac function in patients with myocardial ischemia, provide the basis for comprehensive evaluation of coronary artery disease and its prognosis.

ABSTRACT

Objective To evaluate the diagnostic value of low-dose volume perfusion imaging with 320-detector CT in differentia-ting solitary pulmonary nodules (SPNs).Methods 130 patients with 130 SPNs (79 malignant tumors,1 6 active inflammations,1 6 benign nodules and 1 9 chronic inflammations)underwent contrast-enhanced dynamic volume perfusion CT prospectively.50ml of contrast medium with iodine concentration of 370 mg I/mL was injected at a flow rate of 5 mL/s,and the scan began at 0 second or 4 seconds after the bolus injection.20 or 21 intermittent low-dose volume acquisitions were made with 2 s intervals, and the total ac-quisition time was 38 s or 40 s.Post-procession was performed using a perfusion software on the CT (Body Perfusion,Dual-input single compartment mode). The perfusion parameters (PF,BF,PPI),T-DC and TPF were measured or calculated, and then as-sessed.Results PF and PPI of active inflammation nodules showed significant higher value than those of the other three kinds of nodules, and there were significant differences in PF and PPI between active inflammation nodules and other kinds of nodules. The value of BF in malignant nodules was higher than that in the other three kinds of nodules, however no statistic significant difference in PPI was found between malignant and chronic inflammation nodules. The value of TPF in malignant and active inflammation nod-ules was higher than that in benign and chronic nodules, and there were statistic significant differences between malignant and active nodules and chronic ones.Six types of T-DC were displayed on SPNs:malignant, benign and chronic nodules showed a variety of T-DC types, and active inflammatory nodules showed only one type-typeⅠ.Conclusion Low-dose volume perfusion technique with 320-detector row CT can provide abundant information about blood flow of SPNs,and it may be regarded as an important technique in the differential diagnosis of SPNs.
